Joann Cook

Graveside services for JoAnn Cook will be held at 1 p.m. on Tuesday, September 4, 2018 at Dove Creek Cemetery.

JoAnn was born November 23, 1938 in Missouri. She passed away on Thursday, August 30, 2018 at the Valley Inn Nursing Home at the age of 79.

Surviving JoAnn are her children, Jack Mott (Susan) of Egnar, CO, Vicki Ayers (Jack) of Cahone, CO, Cindy Slade (Max) of Grand Junction, CO, Cheryl Nolan (Bob) of Humbolt, AZ, William Mott (Delilah) of Yellow Jacket, CO, and Tammy Hare or Cortez, CO; her sister, Nancy Boughton of Bath, NY; twelve grandchildren and seventeen great grandchildren. JoAnn was preceded in death by her parents; her husbands Jack D. Mott I., and Floyd H Cook.

Memorial contributions may be made in JoAnn’s name to the local Alzheimer’s Association.
